Medication in the academy
Prescription Medication
Where a child needs prescription medication in the academy we must have a completed form with all the details, giving us permission to administer the medication. The most common forms of prescription medication required in school are items like inhalers, epipens and eczema cream.
Antibiotics that are required three times a day should be taken at home. We will only accept antibiotics in the academy that have to be administered four times a day. These will need to be signed in and out.
What you need to do:

Download and fill in the form below or collect a paper copy from the office

Bring the form and medication in it's original box with the precription label on

We will then check the information and have it signed off by the Principal. It will then be stored for the child's use
If your child does not need to use their inhaler or other presccription medicine in school you will need to complete a medical disclaimer form.

Non Prescription Medication
For any non prescription items such as lip balm and sun cream you will have to complete the non prescription medical form.
